Operations
A management company
for your clinics.
Owning a clinic and running one are two different jobs. Middlemarch takes on the second, and we mean all of it: the staffing, the books, the technology, the building itself. From a broken exam light to the year-end books, the operational load lands on us so the clinical team can stay focused on care.
What we take on
The work we take off your desk
01
Day-to-day management
The hundred small decisions a clinic needs every week.
Scheduling, capacity, supplies, vendors, and everything in between. When the fridge alarm goes off at 6 a.m. or the front door won't lock, that call comes to us too. We put consistent processes behind the daily run of the clinic and hold the standard across every location you operate.
02
Back office & administration
Clean numbers on a predictable schedule.
Billing, claims, payroll, procurement, and compliance paperwork, handled by a team that does it all day. We keep the books clean through the year and line up the accountants for tax time, so you get a reliable month-end close instead of a scramble, and reporting you can actually act on.
03
People & performance
The people side of a healthy practice.
Hiring, onboarding, scheduling, and the performance conversations that keep a practice healthy, plus the operational reporting owners need to see how each site is actually doing.
The whole job
If it isn't clinical care, we can take it on
Running a clinic means being a facilities manager, an IT department, a finance office, and an HR team all at once. When we say operations, this is what we mean.
Facilities & equipment
IT operations
Finances & payroll
Hiring & HR
Purchasing & vendors
Compliance & paperwork
If something comes up that isn't on this list, it's still probably ours. The point of a management company is that you stop keeping the list.
On the ground
Operations happens in the building
Clinic operations is hands-on work, and we treat it that way. Our team shows up in person: walking the floor, meeting the front desk, and looking at the broken thing instead of asking for photos of it.
That presence changes how a clinic runs. Staff flag problems sooner because there's a person to mention them to, small issues get fixed before they become expensive ones, and patients walk into a building that simply works. When a job needs a licensed trade or a specialist, we bring them in, meet them at the door, and check the work before the invoice gets paid.
What showing up looks like
- Scheduled walkthroughs of every location, not just visits when something breaks
- An operations lead your staff know by name
- Small fixes handled on the spot, trades booked for the rest
- Vendors and contractors supervised on site
- A running list for each clinic, worked down week over week

How it starts
From assessment to handover
Handing over operations is a serious decision, so we don't start with a contract. We start with a look at how the clinic actually runs.
